An official Eagle Scout Rank Application must be used. Type or print neatly with blue or black ink. Answer all the questions. Carefully fill in dates of merit badges. The Scout Office will check these dates with their record. They must match. Fill in the unit number where you earned each merit badge. On #6 and #9, cross out the merit badge(s) you did not earn. If you did earn both merit badges, enter one here and put the other in #13 and highlight it. If you earned the Safety or Sports for rank before 4/1/99, put them in #13 and #14 and highlight them. |
